Maayer Therapy is owned and operated by Dave Hanemaayer, Registered Massage Therapist (RMT), functioning as a sole proprietor regulated under the Regulated Health Professions Act (RHPA) and the Massage Therapy Act, 1991.

Dave is committed to protecting your privacy in accordance with Ontario’s Personal Health Information Protection Act (PHIPA), PIPEDA, the Ontario Human Rights Code, and the CMTO Standards of Practice.

Note on Separate Business Operations:
Maayer Therapy is a regulated massage therapy practice and is entirely independent from Ottawa Laser Solutions and 1530910 Ontario Limited. These businesses have separate services, websites, policies, and record-keeping methods. The privacy practices on this page apply only to massage therapy services provided by Maayer Therapy.

1. Information We Collect

We collect the minimum information necessary to provide safe, effective, and ethical massage therapy services, including:

• Personal Information
• Name
• Contact information (email, phone number)
• Preferred name or pronouns (optional)
• Personal Health Information (PHI)
• Collected in accordance with CMTO’s Standard of Practice: Privacy & Confidentiality and Record Keeping, including:
• Health history and relevant medical information
• Clinical notes, treatment plans, and progress notes
• Any information you voluntarily provide to support safe care
• Website and technical information/data including but not limited to IP address, browser type, page interactions, etc. via standard web analytics tools

2. How Your Information Is Used

Your information is used only for purposes consistent with PHIPA and CMTO Standards:

• To provide regulated massage therapy services
• To assess, plan, and document treatment
• To communicate regarding appointments
• To maintain accurate clinical records
• To manage billing or receipts (if applicable)
• To improve website functionality
• To comply with legal or regulatory obligations
• To send occasional clinic updates or practice-related information, which you may opt out of at any time.

3. Sharing and Disclosure of Information

We do not sell or rent personal information.

Information may be shared only:

4. Security and Record Retention

We follow CMTO’s Record Keeping and Privacy & Confidentiality Standards.

• Password-protected systems
• Limited access to authorized personnel (only the RMT)
• Confidential handling of all PHI

Retention Period:

• Records are kept for a minimum of 10 years from the date of last visit, or
• 10 years after a minor client turns 18, whichever is later
• After this period, records are securely destroyed.

5. Your Rights Under PHIPA

You may request to:

• Access your health records
• Correct inaccurate information
• Withdraw consent (except where record retention is required by law)
• Receive information about how your data is stored and used
